The article brought some quite recent memories:
At the time I was taken to a hospital when I fractured my foot two months ago, I also bruised and scratched one hand. After asking nurses too many times to clean and disinfect it ("that's nothing!" they said), I had to do it myself with water.

Tom,
"Strong and vicious insurer incentives" is not a solution he recommends.
You need to read and understand more about Berwick.
